Flapless implantology Inability to maintain keratinized mucosa

A soft tissue punch may inadvertently remove all the keratinized mucosa on the facial aspect of the implant. For this reason, many clinicians consider flapless implant surgery only in areas with sufficient amounts of keratinized tissue. Based on animal experiments, the optimal tissue punch size is 3.0 – 3.5 mm when a 4.5 mm permucosal healing abutment is used.

The peri-implant mucosa heals quickly with little scar formation when the peri-implant mucosa is in tight contact with the implants after implant placement.

These results suggest that the amount of keratinized tissue sacrificed by the soft tissue punch at the flapless implant site does not exceed 3.0 – 3.5 mm in width.

Flapless implant placement can also be performed using a mini-incision technique.

the mini –incision technique is beneficial in saving the keratinized mucosa.
