Flapless implantology Fabrication of surgical guides using a diagnostic cast

a diagnostic cast can be used to fabricate a surgical guide. The diagnostic wax-up is completed to preview the tooth size, position, contour, and occlusion in the edentulous regions where implants will be inserted.

After cutting the edentulous region of the diagnostic cast perpendicular to the ridge, the diagnostic cast cross-section is shaded with a pencil to represent the mucosal thickness. The remaining cross-section of the cast roughly estimates the bone contours under the soft tissue. The ideal angulation and size for implant insertion are determined based on the bone contours. Once implant size and angulation are determined, the implant site is drilled in the template corresponding to the angulation, and then a metal guide is inserted into place.
