Flapless implantology Treatment planning for the edentulous posterior maxilla part two

Block or particle bone sinus graft with immediate or delayed flapless implant placement is indicated when less than 5mm remains between the residual crest of bone and the floor of the maxillary sinus.

To access the maxillary sinus, a horizontal incision for the approach is made on the buccal cortical plate of the maxillary sinus horizontally, with the alveolar crest approximately 10mm away from the crest. These procedures ensure favorable conditions for flapless implant surgery in the alveolar crest. When block autogenous iliac bone is placed, the immediate flapless implant placement in the maxillary sinus may be performed at the same time as the sinus graft. The use of the corticocancellous block bone provides initial mutual graft-implant stability, which facilitates simultaneous implant placement in the posterior atrophic maxilla.
