If the available bone height of the ridge is insufficient, bone augmentation is performed prior to implant placement. After the ridge is augmented, flapless implant surgery can proceed.

The authors advocate performing a sub periosteal tunneling procedure with Bio-Oss block onlay grafting.

This procedure has several advantages over other bone grafting techniques for vertical bone growth. There is no donor site morbidity, little risk of graft dehiscence and incision-line opening, and less postoperative morbidity. After the ridge is augmented, flapless implant surgery can proceed.
